Use consistent identifiers for the various platforms we support.
...
Make sure every source file includes SDL_config.h, so the proper system
headers are chosen.

FIXME:
Change #include <stdlib.h> to #include "SDL_stdlib.h"
Change #include <string.h> to #include "SDL_string.h"
Make sure nothing else broke because of this...
